My practice welcomes several insurance options as well as discounted rates, and courtesy claims submission for cash pay patients.
Below is a list of the plans I am contracted with, and some estimated out of pocket costs. I have a billing specialist who can check the benefits on your specific plan once you have a scheduled appointment.
Medicare & Supplemental Insurance
Some Supplemental plans do not cover the annual Medicare deductible, typically these are Blue Shield of CA supplemental insurance plans, and some UnitedHealthCare. Please check your plan booklet to see if you have a plan that does not cover the annual deductible. If not covered, this is an out of pocket cost.

Med Advantage Plans - PPO plans
If you have a PPO plan, your out of network benefits will be quite similar to your in-network benefits. Many Med Advantage plans do have a co-insurance. My billing specialist can give you an estimate at the time of scheduling.

Commercial Insurance
- Anthem Blue Cross, Commercial plans
Benefits vary by plan.  - Anthem Blue Cross, USHIP (UC Student Insurance)
Please be informed that your school insurance is a secondary to most all other health insurance plan (Medi-Cal, MRMIP, or TRICARE). Below is a link from UC Santa Barbara explaining this. You are required to coordinate your benefits with your Anthem USHIP plan and your primary insurance plan. My billing specialist can walk you through this process.
Sutter Health HMO Department
If you are a patient at Sutter Health, and you have a HMO plan, you can work with your Primary Care Doctor, or referring providers office, to ask that an authorization be created within the HMO department, which can cover your testing appointment at 100%. Below is a list of the HMO Health Insurance plans handled by the Sutter Health HMO Department.Â
